  1. 1. Log into your 403(b) Plan account by clicking here.
  2. 2. Speak to a representative by calling the Advocate Benefits Service Center at 1.800.775.4784 and pressing 1 for Retirement. Representatives are available Monday through Friday, 8:00am–6pm CT.
  3. 3. Complete the 403(b) Plan distribution form by clicking here.
